If you are a Support At Home (or CHSP) coordinator or NDIS support coordinator, you can easily refer your clients to us for in-home or in-clinic services. Simply use one of the following methods to make a referral:

Fill out our online referral form
Download a referral form, fill it out and return to us via email or fax.
Support At Home (SaH) and NDIS funding can also be used to access our group services, which is a great way to maximise services within a budget.
Whenever we receive a new referral for an NDIS participant, we put together a service agreement, which we then send back for review.
If you are a GP and wish to refer a patient to Leading Therapies for services through a Team Care Arrangement of Department of Veterans’ Affairs funding, you can fax our office on 08 8172 2555.
